Payment & Insurance Options at Elizabeth Brunson Counseling

Payment is expected at the time of service unless otherwise agreed upon at the beginning of treatment.

Note regarding Insurance: Most persons would like to utilize their insurance for mental health services, but health insurance plans and coverage differ widely.

You may use this information in this document to contact your insurance provider about your current benefits and to determine in-network eligibility:

I currently accept and will file insurance for the following companies: BCBSNC, CBHA, and CIGNA.

Co-pays will be due at the time of service, and if my services are out of network with your insurance company, I offer a super bill with a detailed record of services, which you can submit to your insurance company for reimbursement of these costs, if your plan offers this coverage.

Good Faith estimate notice to clients and prospective clients:

If you DO NOT have health insurance or do not intend to file for reimbursement from your health insurance, you are legally entitled to an estimate for the total expected cost of services you will receive at Elizabeth Brunson Counseling PLLC.

The federal “No Surprises Act,” implemented in 2022, ensures that you have the right to receive a “Good Faith Estimate (GFE),” explaining how much your medical and mental health care will cost before you schedule any new or recurring services.   You will receive one before your first appointment. Since a diagnosis is required for the Good Faith Estimate, regardless of whether you intend to submit your superbills for insurance, you may elect to “opt out” of Good Faith Estimate requirements if diagnostic privacy is a high priority for you.

Feel free to ask any questions that you have about this before our first appointment and note the following guidance:

- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

- Make sure to save a copy or picture of your Good Faith Estimate.

-For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises/consumers or call our office at 310-6833-9047.

-If required, you will receive one as part of your intake paperwork, via the online client portal.
